You could do it a couple ways. In the past I've done it as two individual dimmers, the advantage here being that it each channel can be assigned to submasters. I've also done it as a profile with intensity controlling haze volume, and then the control channel controlling blower speed, which is more consistent with normal lighting fixtures. It's really kind of just personal preference.

You could definitely create an effect that'd turn the hazer on and off over a given interval. I included an image of one way to go about that. The "On Time" fader controls the percentage of the cycle time that it stays on, rate controls the speed of the cycle, and influence controls the amount that the hazer and blower will turn on at.
